2. Never used PD as frontline unit, dont know. Seems odd.
3. HM/OCC/BH/MAA. Mark skils on HM and OCC, Hound hury, heal on occultist, stun and both melee skills on BH, and MAA with stun and guard. This is main skills, rest isnt relevant. For trinkets i took shield with +stuns for MAA, and for others some good stuff, like speed, or sun rings. Works normal, even with underleveled heroes.
plague doctor is a great row 3 or 4 for DD3 because her blinding gas skill stuns both the teleporter minion and the big boss guy (since he takes up ranks 1 2 and 3) so i think PD is good for that dungeon just not rank 1 or 2
oh and plague grenade blights the teleporter minion and the boss guy so another reason for her being great in DD3
as for party comp last time i did it was NG+ and i used ARB/OCC/HM/CRDR. so arbalest would have two damage trinkets, like the ancient musketball and a sun ring or something. the occultist or houndmaster would mark the target depending on if it had dodge or PROT. then the crusader was a stress healer and stunner with a stun trinket. i would mark the teleporter guy with my occultist, usually one shot it with my arbalest, then the houndmaster would keep attacking the big boss guy while my crusader stunned him whenever possible.
in my current game im probably going to end up using PD/VST/HM/HEL cause PD can blight and stun both, vestal for healing and backline damage, houndmaster for stress heal and backline damage, and then the hellion for YAWP, and iron swan. this party is probably better since it is less mark dependent and yawp will make life much easier going through the regular fights before the boss fight
one last tip: the trinket "Ethereal Cross" is pretty much a must have on almost all party members (except vestal for double heal trinkets) for DD in my opinion. it gives bleed resist, which a LOT of enemies cause bleed, and increases eldritch damage. im pretty sure every enemy in the DD is eldritch so it's by far the best in slot trinket for any damage dealer on your team to have.
