When your kid is born, the game determines what happens - a health baby is born, two healthy babies are born, the baby is stillborn, or your wife dies in childbirth (healthy baby). If you're really worried, uou can load a save before the birth if you get an outcome you don't want.
No don't dismiss, if you are in the inn or in the keep in top right corner next to portraits of characters that are there, you can use click a plus sign for an option "leave character here".
I don't think leaving your wife in a town will keep her safer though. But I guess it depends how you want to role play it.

All is a die-cast probability
As governor/guest in a settlement:
Pregnant wife can still die during birth and/or loose the child.
The child can survive even if his mother dies.
all the above have LOW probability, most of the time all is good.

As a fighter, on her own or at your side:
1 Even if not hurt and kept behind, it has a chance of loosing the child/ die in pregnancy: a medium/high chance
2 If wounded and/or has to recover:
Highest chance of death of both mother and child.

Solutions:
Remember the time she got pregnant.
Save before the date of birth.
Do not do anything important, live her in a settlement (any settlement), you just moving around, auto-safe small battles, and...
If death is the result;
reload until is all OK
could be some 5 to 20 reloads.
But she and child will be OK !
