Using 1920x1080 in-game with a 1920x1440 desktop results in stretched image
I'll get right to the point, it appears the game might be assuming that a resolution ending in "1440" must be widescreen, and is resulting in a 16:9 resolution being stretched to fit 4:3 in fullscreen.

My monitor's desktop resolution is 1920x1440, however more and more games are designed only with widescreen resolutions in mind - which is understandable - therefore I run some titles in 1920x1080 letterboxed (black bars above and below) since such titles weren't designed to run in 4:3. I'm okay with this arrangement.

However Gauntlet is not behaving. When I attempt to launch Gauntlet in 1920x1080 with a desktop resolution of 1920x1440, the game's render window attempts to stretch to fit the entire 4:3 aspect of my monitor. Yet when I check the OSD scaling, I'm still using my 1920x1440 scaling settings, meaning if I adjust the scaling to add black bars, those black bars remain when I return to my 1920x1440 desktop. The game is not properly changing my screen resolution to 1920x1080 before launching.

This problem does not manifest if I change my desktop resolution to 1920x1080 and launch the game at 1920x1080, it also does not manifest If I use different resolutions, such as launching the game at 1280x960 with a desktop resolution of 1920x1080; the game creates a proper 4:3 render window in this instance.

What do I need to do in order for this game to properly adjust my monitor's resolution to the one I have selected in-game before actually launching the game and attempting to stretch the render window to the desktop resolution's aspect ratio?

You can try manually setting a resolution in the config file, though I don't know if it will work. The config file also has an aspect ratio setting that you might want to fool around with. It is set to -1 for me, so I don't know what format they expect, but you can probably figure out the 4:3 and 16:9 aspect ratios with settings you have already done, and manually mix them with different resolutions to see if you can get it working how you want.

Thanks ChevyNoel, but I'm already setting the resolutions manually, as the ones I actually want to use don't appear on the menu in-game (it seems as if they overflow beyond what's visible, not good UI design).

I have toyed with the aspect ratio setting, however it doesn't change the game's behaviour of not correctly setting my monitor's resolution before launching the game; no amount of aspect ratio fiddling will prevent the game stretching to fit a 4:3 monitor while using a 16:9 resolution.

I meant you could try setting a resolution they don't have in options, like 1920x1440, to see if it will fit your monitor, or set the resolution to 1920x1080 and set the aspect ratio to 4:3 to see if it letterboxes.

Perhaps I wasn't clear; I have already tried everything you have stated, the game is making an assumption it should not, or not performing an action it should.

This is not something that can be fixed on the user's end.

How can this problem be solved when the game is being played on Geforce Now?
