In usr_input.ini within documents/capcom/re4, replace the MA_whatevers with keyboard keys of choice. Leave the zero fields alone. Just tried it in order to verify this mouse aim horsecrap and lo and behold, it works. So at least the method they went with is good for pointless ways of aiming.

am stuck at figuring out how to get reload only while holding shift
and there is also too much sensitivity while aiming not sure if its only with me or u have same problem
just found solution for sensitivity
for people using re4 hd project press f1 in game and change mouse tweaks some of them affects aim sensitivity
eventually we can go for compromise and reload on r so we can run freely without reloading first
nvm reload is also a matter of tweaks on f1 this time in keyboard settings
