You can't miss it in act 2, just complete the act it tells you exactly where to go.

Act 2 in the ghost world/bedroom telescope? it's required to beat the act and earned in that act.
Edit: Just checked it's definitetly act 2 in chapter 3.

To be completely freaking obvious if anyone else gets here... start ghost world act 2 and
you'll find a platform with a bell. Ring it by hitting it, then jump up the leaf platforms and into the well at the top.

After that you literally can't miss it because the game won't let you out of the well without it.
