Your Processor is your weak point, it's responsible for your frame drops and reducing graphics settings likely won't help since most of them don't strain the processor to begin with, might as well keep them high.
Lag could mean a lot of things and it's a hard thing to describe, but you might have to try.
Is your old TV a Plasma TV by chance? I've played on a couple different plasma TV's and always found they had an amazing smoothness to them that you can't get on any LCD.
Next, it's important to understand what each thing is doing. V-sync syncs your card output to your monitor refresh rate, so, since your monitor is 60hz, which essentially means it outputs 60fps, trying to output at 130fps means more than half the frames are being thrown away, which means your system is working twice as hard as it needs to.
Just set your FPS limiter to 60 and turn off V-sync.
Everything should be fine and look better.
If not, maybe you have a background process (a virus/trojan) that you need to take care of
What im wondering is, that the problem comes since i got the new monitor, and only on PC, on XBox One i got nothing.
Turn off discord/xbox.
Medal recording is probably causing problems. You need a better processor if you want to game and do a lot at once.

Also, of course, try it with a different TV/monitor if you can and if you buy a new cable, you might prefer to get a diplay port cable, instead, since that will perform better and I believe that your new monitor and your graphics card both have the ports. Make sure to check first though. Good luck.
