Win trading aka. Players ( like in my case in 2s) queue up at the same time with 3 players , so they get into one game. 2 of them are on team orange as premades, the 3rd is matched against them with a random(me). So they team up and play 3v1 until you give up and forfeit.

What you can't get into the same game with people on opposite teams.

2 of them are in party, the 3rd queues up seperately. In champ 3 don't queue so many people at this time, so its pretty easy to set this up.

It's actually not, and it happens even in Gold ranks, although a lot less often.
