After sitting in the car after the first mission, with no hint or clue as to how to actually choose the "start car" option and trying stuff for a few minutes I quit the game and checked the time played to see if I could still get a refund.

Is this some sort of joke ? What's wrong with using one of the standard kb / mouse binding sets ?

Normally, the regular action buttons (pick up, attack) also let you select any option.

Additionally, the vast majority of games allow you to remap keys. You could simply have gone into options and bound the actions to whatever keys you wish.
