So I wouldn't be surprised if something deep in the belly of the beast is causing it.
At least for me, I am done attempting to use MMB for anything in Rust apart from stacks. This caused me to have to reset my binds to default which sucked cause I have many custom ones.
Well, that's one way I can get access to the Mouse controls for vehicles... haven't been able to figure out how to do that naturally and everyone that has ever mentioned it doesn't seem to have a verifiable method for enabling it... Had it happen by accident a while back and actually really enjoy the fine tuned turning for Motorbikes.

Okay so, notes about what I've discovered just in case anyone cares, or in the case that this information might somehow assist the OP.
I can confirm that MMB is not bound to ANYTHING by default if you reset your keybinds. As long as it stays that way, you can access Fine-Tuned steering by pressing MMB while driving a vehicle. It reverts to WASD steering as soon as you release the MMB.
I can also confirm that if you bind MMB to the "Switch Zoom" function, it causes land vehicles to get locked into this mode by default and you cannot access WASD steering by holding MMB. Being in this mode does not seem to affect Horses, Boats, or Aircraft.
This seems to support @No_Quarter 's assessment that it probably has something to do with the inherent macros in the game's code.
IMO, they should just make it so that MMB is not able to be bound to any function for the time being to prevent people from encountering these sort of bugs. I've seen these sort of discussions startlingly often, with a great deal of frustration from the community surrounding these issues.
A list of the currently available macros, like in the game's options... might be nice also, if they can't find a way to let people rebind them outright. A lot of the discussions about the issue wouldn't even be happening if people had easier access to understand the more complex controls.
All that being said, I hope that whatever happens, I can still get to steer like this on purpose. Makes the motorbike feel like it has way more potential for high-skill driving. Just imagine pulling up with a gang, using sidecar bikes at launch / radtown to just straight up hit and run people.
