How and where you can get Achievements?

I mean, cani get all the achievements by playing offline? Or i'm forced to play online?
If is online, i need to play in official servers or community ones are good too?
A PvE server is enough for get the 100% of the achievements?

Most achievements can only be unlocked on official servers, and yes, you have to be online.

Whether it needs to be official or unmodded server actually seems to vary randomly over time, probably because facepunch code is inconsistent and buggy as all hell... and Steam is no better... I've had achievements on modded servers at times, and also failed to get award when task completed on official. I've seen the same random results with other games as well.
