No, there isnt a sex option. There probably never will be and the games age restriction (if there is any) will be raised significantly.

If you mean to change your gender:

Well yes it does.. if your wife/girlfriend/lady friend for the night sees you playing this wonderful time consuming game then the option of sex is still there.. depending how long you play is to wether you get sex of not from "She Who Waits".
