As for your second question...I dunno, but it doesn't really bother me. I literally never finished the game at It Lives after unlocking later levels in the original, I'd always prefer to keep going until the very end.
There are situations when going further is meaningless, due to bad items or low health or anything alse. Finishing the game after It Lives was a good opportunyty to finish a run prematurely.
No run's ever meaningless. I've had some situations where I thought for sure I woudln't make it and then I either managed to do it through some serious skill, or some insane luck, or both. Or I just die on the first room of the Cathedral/Sheol, either way's fine with me.
Perhaps they didn't want players ending the run early? You can still end it sort of early by taking the wrong photo and going to the opposite path.
Anyway a player have -1 boss item, because of the Polaroid/ Negative spawn istead of normal item. Not to mention boss rush runs, where you don't go enen to Womb. Spawn Polaroid/ Negative with a boss item like in the original TBoI would be great, imho
You go to the womb on boss rush runs. After you finish boss rush it gives you another extra item, then let's you leave and head on down to the womb.
The polaroid is in itself a fantastic item depending on the situation. If you have only spirit hearts it activates whenever you get hit, same with the Negative. The single missing boss item doesn't really hurt all that much.
Picking up the same item on every run is boring, at least for me. But polaroid is esential to game progress, and have to appear after mom fitght. In original game players were crying about its dropping chance and devs make it appear on every run with boss item. So give an opportunity to collect something else beside it and everyone will be pleasured )
People wanted the Polariod to be changed, because it being a trinket and required ruined the trinket system to some extent. They decided to make the Polaroid/Negative items instead, so you wouldn't have to give up your trinkets. So, I guess to balance that, they made it so you wouldn't get the boss item from mom.
A single missing boss item doesn't make a huge difference most of the time. If you're bored of picking up the same item, pick up the other photo instead. I just don't see any reason why it would have to be changed, it has barely any effect on the game, and is fine as it is. The Polaroid/Negative is your reward for beating mom, you don't need other items on top of that.
So make it exagerrated, Let's imagine after every boss fight player pick up the only one defined by devs item, You need only 18 items at all. Pretty easy to balance and add synergies but it's boring ) And it's boring to pick up only polaroinds/ negatives after mom fight not to mention that rouge-lite RND-fun is decreased by such design decisions.
For me picking new random items is the one of main parts of the game. And because of that Womb "balaning" by removing items rooms makes the stage quite straightforward and uninteresting, You have only to reach the boss and have no reason to explore, exept to find a few hearts.
