What is this?
I don't know what is it, i think polaroid/negativ but the rest don't.

Those are completion marks, they show what you've beaten with each character.

The heart is Mom's Heart (or It Lives).
The star is boss rush.
The cross is Isaac.
The inverted cross is Satan.
And since you already identified the Polaroid and the Negative I don't think I need to say what they signify.

Also they show if you've completed that "task" on normal or hard mode, you have the normal mode completion symbols, in you do it on hard mode the symbols get a red outline.
