Maybe has something to do with Lazarus? I got both the other day and I got my 9 lives counting down when I lost them.

Yeah sounds like a bug but since it didn't happen to me with a different character I was thinking maybe just a bug with Lazarus and the combo with his revive. It's not an intended design change with the 9 live and collar.
i have no idea if any of the devs read this but i am hoping that there is a patch coming at some point

Before that, you can also try re-creating this situation, using the debug-console[bindingofisaacrebirth.fandom.com], which can help with troubleshooting.
Also, i'd suggest giving the link posted by Nazemec a visit to better understand how extra life items interact. Under normal circumstances, Guppy's Collar cannot possibly trigger, before running out of all lives from Dead Cat first. So either Dead Cat was bugged and didn't trigger or was removed somehow before.
Did you by any chance had a re-roll item, like the D4 or D100 on you? If you also held the Panic Button-trinket, Dead Cat could've been re-rolled, before taking fatal damage. Repentance changed Dead Cat to be properly affected by character re-rolls now.
