It’s easily the single most over powered item in the game now and guarantees a won run like almost every time.
You can unlock it by defeating Deirium as Tainted Bethony.
It's found in Devil Rooms and Libraries
She also starts with Lemegeton as her active item.

Probably an unpopular opinion but I really don't like this item. Pretty much everything is going to kill your wisps, I prefer to just have permanent items.

they take about 4-5 contact hits and a few projectile hits. even moms hand or foot stmping on you doesn't get of even a quarter of them

Yeah everytime I play as tainted Bethany I just steamroll every run. Imagine getting Godhead, tech X and psy fly as wisps in the first two floors, happened to me and I didn't even need to take pedestal items to win.

most of them can if you're good enough ;)

but literally urn of souls carries a run by itself because there's literally a challenge where you literally just win with urn of souls by itself and it's super easy and not a challenge because that's how good the item is

literally spirit sword can carry a run because it did for me when I had 2 damage and low tears and T.Maggie and it let me kill Mother.

Psy fly can pretty much carry a run all by itself

Dark arts, berserk, revelation, psy fly, etc can carry a run to a degree. They make otherwise awful runs bearable and winnable.

red key, stairway, rock bottom, echo chamber, r key, glitched crown, etc pretty much guarantee a win, of course not all by themselves but no one plays isaac and only picks up a single item. Synergistic items are usually the most fun anyway!
