Habit + D20/D∞ + Blood Donation Machine/Demon Beggar. Bonus points for Schoolbag + IV-Bag.
The idea is, to create lots of money, re-roll it, open all chests, pick up enough red-hearts (and batteries, if any spawn) to re-roll again. Most of the time, you'll end up with more pick-ups, after opening all chests and some chests will contain items. There are more variations of this. Some use different pick-up generation engines (Fanny Pack, Swallowed Penny, Old Bandage, Endless Nameless/Blank Rune. Glitter Bombs+Dr Fetus/Curse of the Tower, etc) and others use Scapular and even the Cursed Skull trinket (in conjunction with Scapular). There's a tons of ways to create infinite pick-ups, but finding a way to re-roll them over and over is key.
For greed mode, you only need the D20 and the shop selling a battery to achieve this game-break. It's extremely easy there. Blank Card+2 of Diamonds still works there as well, due to how shop items still re-stock in a pre-Repentance fashion.
Clear Rune+Jera is another easy game-break, if you manage to duplicate enough batteries. For this game break, you need at least 3 battery pick-up's with a full charge of Clear Rune, so you can start to create infinite amounts of those. Then, the next time, you get a golden chest on that floor, duplicate it, before opening it. Open only one and if it's an item, duplicate the other one as much as you like for infinite items.
There are also some semi-game breaks, if you get lucky, like Dead Cat+Blank Card+Suicide King (especially in the secret or angel room), Crooked Penny or Diplopia shenanigans, like using them on R-Key with Schoolbag or better, Moving Box, to carry even more R-Keys, if you get lucky with Crooked Penny.
You can also use Schoolbag+Glowing Hourglass+Habit for a proper gamebreak with Crooked Penny, but since Repentance, the result will always be the same after using Glowing Hourglass, so you need to use it in another room first, in order to try again inside the room, you want to duplicate items at.
Battery Items can also spiral gamebreaks like this even further out of control, Car Battery in particular. Oh and Damocles, but you better hurry, have Gnawed Leaf or an extra life.
Tainted Cain itself is also super broken. He's not everybody's favorite, but he's also gamebreak incarnate. Crafting Portable Slot is a fixed recipe and requires 8 coins. Crafting an active item destroys your current one and spawns new pickups (which i think still depend on the room you're in, so do this in the secret room for Bone Hearts). You could, for example, craft another Portable Slot and i think you know where this is going. It's possible to do almost all his checkmarks on a single run (technically), once you get the recipe for R-Key. And if you craft The Clicker, well, that's a bunch of easy T.Jacob/T.Lost unlocks, if you struggle with these characters.
Edit: There also are a couple ways, to achieve immortality. I already mentioned Scapular+Cursed Skull for a D20 gamebreak. But if you add The Wafer, you basically can't die. Another classic way, to become immortal, is Pyromaniac+Dr.Fetus/IPECAC/Curse of the Tower/etc. If you're patient, Super Magnet+Gnawed Leaf+Orbital/Contact damage.
But probably the most consistent way, is Book of Shadows+4.5 Volt (or Clear Rune+Algiz+4.5 Volt) with good DPS, which allows you to just spam the book vs. bosses.
Note to self: unlock wild card
Red Key + Book of Revelations, where wisps generated spawn red key wisps that have an extra chance of opening a red room upon room clear. Red room generation becomes very powerful at max capacity.
Glitched Crown + Chaos, when the former item is already a run-winner on its own, it's much easier to obtain any specific item in the game. No farming for resources required.
D7 + 4.5 Volt, on the Chest/Dark Room where all chest rewards are items, all it takes is a large room with tanky enemies to charge this item over and over. Example being the triple Super Envy L-room in the Chest. Otherwise, a decent infinite albeit slow room reward farm.
Deep Pockets + Options?, not necessarily a gamebreak but an extreme resource generator. When Deep Pocket's effect procs, generating 1-3 'pity pennies' on missing a room reward, all of them will get duplicated by Options?. It makes it so chest generation is on par, maybe even better than the Perfection trinket.
There isn't truly a common and powerful gamebreak seen in Repentance compared to AB+ with the double steam sale restock or blank card Jera. However there's a lot more run-winning combinations compared to breaking the game wide open with many resources and items. Feels a lot less samey, while rewards knowledgeable play on how many more items interact.
With Pound of Flesh+Restock, Blank Card+Strength is also a gamebreak on all characters, if the shop sells a battery, but note that leaving the room will substract all temp HP ups from Strength, but you always keep 1 HP (you can lose heart containers by spamming a Strength-card in the same room).
Pound of Flesh+Restock with Binge Eater can also give infinite stat-ups.
D20 tricks are also still very much viable and made all the stronger by the new D-infinity allowing you to just pick and choose your dice, turning it into the single greatest multi-tool in all of Isaac. It's just a matter of where you wanna abuse this trick and the answer is most invariably the Secret Room. Secret Rooms are a cornerstone of game busting in Repentance, because what was already a reliable pool of staple items is now a cesspit of some of the grossest items in the game. Glitched Crown, Echo Chamber, Rock Bottom, R Key, each of these items can facilitate easy GGs all by themselves, so getting access to them ought to be your priority.
The Void is also super useful for these purposes, as while most of the duping items like Jera take massive charges from Clear Runes, Void entirely ignores this rule and you have once-again access to 6-charge Jeras.
