If you can get items or trinlkets that protect you that's great. Obviously you want blank card on this character and any items that reduce the cooldown for it. I actually never use the holy card right in the beginning I wait for blank card to activate or (or if I find a second holy card) Backpack and book of shadows are also great stuff to have aswell as wooden cross.
I play T Lost exclusively and there is a consistency to this character when you think about it. It's bizarre. Since no defensive/health items exist, devil rooms are great again so go for those. Early security even, with potential run-winners like Damocles or Legemeton. Taking every second floor of a chapter will guarantee a boss challenge room that might be worth the risk. As well as going through the mirror world for extra resources and a boss item after picking up the Perfection trinket. You won't be strapped for stats by doing everything.
Shops are one of the few ways to get defensive with card related items or Glowing Hour Glass. Libraries are one of the most important rooms for its great actives. Wouldn't blame you if you reset the first floor until you find one on the starting room. You can also pause the game at anytime to analyse a situation or quit and resume to reset a tricky situation. Lastly well, it's easy to get stacked with this character but your own error runs the risk of a quick death. It's comes down to playing it relaxed and knowing what enemy patterns you need to work on. Oh, and no explosive items like Ipecac. Not even skill can predict those situations.
One tip what I´ve done! Repeat your first floor to find a good starting item, take the risk if a cursed room apears to enter. Not unlikely you will find one more good starting item in there. Cost your holy card though. Maybe you will get another one later. And it is more fun, because of more power right from the start. Just be careful you will die from one hit in a while.
Little horn's burning bombs champion variation, it will sometimes summon the bomb onto you which is usually not a problem but the bomb deals damage on contact.
Shockwaves in small rooms or when for some reason you cannot get really far away from the enemy doing them
The falling spikes in the last phase of beast also seem inconsistent unless you have good movement speed.
I sometimes cannot dodge the heretic's dash, it's just too fast for default move speed to handle.
I am also annoyed by just how many items can get you killed in this game. Saturn. the one that gives you a ring of tears can detonate explosives and enemies that shoot bullets on death. Rainbow cake is generally good, but it can teleport enemies around and cause some other bad item effects. Lode stone, strange attractor, ocular rift yank bullets and enemies around which can get you killed, usually when fighting a bullethell boss(most final bosses in the game now).
A thing i wanna add about delirium, it is not always just the telefrag, sometimes it turns into a quick version of a boss and just dashes onto you in a way you cannot avoid etc. the boss is a complete mess with no patterns.
There are probably more that I cannot think of because usually taking a single instance of bs damage can be shrugged off and also because of how quickly everything's happening and how your character's ability to deal with certain situations vary from run to run, the lines are sometimes blurred between something you can avoid and something you cannot.
Higher damage output = less opportunities for you to get hit.
Unless you are planning to kill Mega Satan or enjoy going through the first 4 floors with pisspoor stats, you should always visit the first devil deal, even if you don't have enough health to afford anything. Their room layouts frequently have soul/black hearts lying around and a lot of items you can buy there grant health upon pickup. If the first deal doesn't have anything good, then you can think about going for angel rooms.
Hell, sometimes I buy stuff from the Devil even if I'm going for Mega Satan. Since good devil deals make you powerful enough to almost never get hit, I almost always have enough health to spend on sacrifice rooms and get the key pieces through them.
