not seeing it lives

i have it iunlocked but i just fight the heart

According to your Steam achievements you haven't unlocked it yet.

Edit:
To be more precise, your latest Heart-related achievement seems to be Experimental Treatment, which is the 7th Heart kill. It Lives is unlocked at the 11th one.

You probably assumed It Lives was unlocked by the "Everything is Terrible" one (5th Heart kill), but that one just makes the game a bit harder. Here's the detail of what it does:

Was it during a daily run? Those are played on fully completed saves so that all players are given the same chances. Never refer to those regarding unlocks.

If you are starting a new save, by any chance do you have any mods enabled? Game requires ya to complete a unknown number of runs which i forgor without mods being enabled otherwise the run won't be counted and no progress to be made

You have to do the first 11 "It Lives" kills without mods, but after the achievement is unlocked, you're free to install anything you please.

It's only one run, you just have to unlock the Womb.
OP already unlocks things and they are way past that point.
