Yes, it's a single player game. Yes, you can (and should) still balance a single player game to make it a more tight, skill based, and generally consistent experience.
- bug and crash fixes
- fully implementing Birthright, adding effects to remaining horse pills
- removing items from greed mode that don't do anything there or giving them an effect
- placebo + things like shot speed up don't take 12 rooms to recharge
and maybe more things I'm forgetting.
Balance changes aren't something you can hold to ransom until other balance issues that you prefer are addressed, my dude.
Bugs will still be worked on.
Missing content remains in the pipeline.
Greed has needed an overhaul since it launched, I wouldn't hold my breath.
They can still adjust Psy Fly while all of this is true
No it's not. What about enemies that rush towards you, explosions, brimstone shots... ?

Ah, I forgot. What some players want, is that the artificial difficulty of this DLC remains in any case.

There are plenty of other items that can also trivialise bullet hell segments, like Censer/Nightlight, Stop Watch, lots of orbitals, Book of Virtues in general, Lost Contact with a good tear rate, (and I would argue Butter Bean with repentance's buffs, shoot me) ecetera. Psy Fly is a very good item but it's not abnormally good.
At some point it's going to be fixed and I'm sure this fix will be misconstrued as a nerf. Can't wait.
