How you guys manage to play Jacob and Esau?

They're completely ass to play as, more for me than playing the lost with holy mantle, they're feel completely unfair when every bullet is basically punishing you for existing, just because you could't have any possiblity to dodge that. Any good items on them or tips?

a good strategy to play with J&E is to restart the run until you get a floor with a library.
If that room has at least the book of revelations or the satanic book you can give it to your most vulnerable character (usually Esau) so that he can accumulate health.
Not to mention making sure one of them takes the books first so it can have the bookworm transform.

You can choose the alternate paths to guarantee yourself a double item from the treasure rooms.

It is also easier to accumulate pick ups, find secret rooms and destroy tinted rocks or extinguish blue fires because you can put 2 bombs instead of 1 without spending 2 bombs

Finally a strategy that has occurred to me but that I have not tried is to have Crooked Penny and Glowing hourglass, try to duplicate the items with crooked penny and if it fails, you go back in time with glowing hourglass until they duplicate.
It is worth mentioning that when you go back in time, you must waste crooked penny and reload it for a new try.

PS: if you have the sun card, make sure it is held by the character you want to be affected by said card, since contrary to what it seems to imply, it does not have a global effect with both characters, which means that only the character who used it will be cured

They are a pain to play, I don't know if it will help you but here's some advice.

First off, you want to focus on making one of them your carry; Jacob is preferable since in some niche situations you will need to control him. Health needs to be distributed evenly.

Use Esau as a Guinea Pig, make him eat all the pills you can find and undesirable items. After some time of doing this you should know most pills which helps a bit on the run.

Always try to stack them in a line facing the boss, makes dodging a little less painful and in a crisis you can make the healthier one tank for the weaker one.

Use your doble bombs wisely, you can check 2 spots for the secret rooms, open up pick ups and deal some good damage with only one bomb.

I know this seems to clash with my first statement but you really need to know how to split the items, even the good ones if they won't work with your main damage dealer or if it's insignificant. Like giving 2 damage multipliers to the same twin or giving luck based items to base Esau.

All kinds of trinkets are good with the twins but I recommend blind rage for Esau, gives you some time to fix their positions.

Oh, and don't get flight items unless they come in a pair, makes dodging an absolute nightmare.

Some extra bits I forgot, you can try to do the alt floors for the extra rewards but I think you should only enter downpour and mirror world, mines are really difficult with the brothers and mausoleum is only worth it if you feel confident in your strenght, if you need to go to the mines for Mother, try to enter Mines 2 so you don't lose too much health.

Always check the shop for the "Options" series items, they are wonderful since you can have 2 full build Isaacs, presuming that you got at least one of them early on.

Lastly, look for ways to use the twins separated health as an advantage, curse rooms and sacrifice rooms come to mind.

I hope at least one of my tips makes your J&E experience more bearable.
