Report a translation problem
The D100 combines the effects of several items:
- the D6; which rerolls all pedestal items in the room.
- the D4, which rerolls all items you currently have (except itself).
- the D20, which rerolls all pickups in the room (coins, bombs, keys, cards, chests...).
Everything seems normal (D6 effect triggered by the D100). Nothing forces active item to become other active items when rerolled (same for passives into passives), the only restriction is you'll get an item from the current active item pool (the one corresponding to the room you're in).
So, just tested the thing. The active item (whatever it is, even if it's the D4 or D100) definitely transforms into a random passive (most likely because it fails the "am I responsible for this reroll?" check), but I couldn't replicate "seeing the old active in the recent items". So I'm not sure why it did that for you.
Might have to try by giving myself a lot of items first, I think sometimes the self-reroll effect can fail. Maybe it could be related (as in: active removed to reroll, can't find what to reroll into so doesn't change, but active still removed yet still owned).

I rerolled myself a bunch of times until I've emptied item pools first. So I think my previous theory (in parentheses) isn't that far of the mark, haha.

Wait, no. The D4 actually still got rerolled into that blue-fly-can't-bother-to-find-exact-item-name-because-that's-irrelevant item, I didn't have it before activating the dice room.

Might've dreamed in Edit2, just did the whole process again twice several times and didn't get a random passive, meaning the D4 wasn't rerolled into anything. I guess it means my initial theory stands?
Managed to get all actives I tested with as non-functional passives: Mega Blast, Void, dice items, books...
