Why did I die in this picture ?

As you can see, I have 2 full soul hearts, I just entered the Chest and I have the polaroid, does anybody have any idea why I died ???
No, I did not have Damocles, No I did not have 9 Lives or any other revive, no I did not take damage by the boss before going down into the chest.

What's really alarming here is that you still have the two soul hearts. This means that by definition you weren't dealt damage.

What character were you playing? If it was tainted Eden then this death is normal since he dies with hearts very often.

All I know is I'm pretty sure that head icon is for when something kills you that normally can't

Dying from self-inflicted damage or suicide items when you haven't taken damage from any other sources will have Isaac's face showing on the page.

Did you have suicide king or something

No I did not, I literally just entered the floor, I had the one card that you can see on the bottom right

Exactly, and I have 2 because my eternal heart even gave me one before instantly dying out of nowhere

The Birthright effect for ??? is:

Soul hearts gained from health ups will be doubled. Health down pills will remove 2 soul hearts instead of 1, however.

This works for eternal hearts, so the fact that you had 2 soul hearts at the start of the floor seems to imply that you had 0 hp during the transition to the Chest. What was your health like when you left Cathedral?

I think I had half a soul heart with one eternal heart active but it wasnt clearly visible, it might have also been a full soul heart with an eternal heart

I thought that this was caused by the fact that your last heart was the one from scapular and it got bugged due to the extra health from your eternal heart, but that works fine. Did you have any other notable health-related items? Maybe you used a strength card before jumping into the chest? Missing No.?

My Theory:
you had lil' chest which can create a pickup when clearing a room, you had only half a heart + eternal heart and it spawned a troll bomb, it exploded while you were getting in the chest, the damage got registered but too late to be displayed because of the chest animation, you then got to the next floor which automaticaly procced your eternal heart (+2 soul heart whith birthright and blue baby) then, after it procced and before the soul hearts were registered as part of your health, the registered damages procced on your last soul heart, killing you.
The screen then displayed your two soul heart but you were already dead, and since the damages dealt came from your bomb on another floor the game considers it as suicide, displaying Isaac's head.

Not sure about the lil chest dropping troll bomb part, might have been something else but that's what I think happened with some kind of damage, lil chest is just one way to explain it.
I might be totally wrong tho.
