Marked skull doesn't give The Fool

The marked skull gave me a sack. I understood it should give me The Fool card?

bombing the x skull has always dropped a fool for me at least, so i suppose its got something to do with ur build

So I guess that's a bug. I doubt it should reroll the Fool card to a sack.

Considering that the marked skull exists solely to give players a sure-fire way to leave Mom's boss room, it definitely seems to be an unintended effect.

Also make sure to not take Little Baggy if you're going for Dogma. For obvious reasons. I think it should be made so if you have Little Baggy, the skull drops Telepills. And with Sack Head, the card should just not be affected by it.
