Worst item in terms of what it does to Isaac?

At first I would say something like the intruder or 8 inch nails but after seeing how the kidney stone works, it's probably the first time something made me genuinely wince in this game. How can someone even think up of something so awful like that?

I dunno, man. Passing kidney stones definitely sucks, but having a spider gouge out your eyes, take refuge in your gaping mouth, and blow your head apart as it exits seems hella worse. Chemical Peel would probably be up there too, as well as the contortions presumably caused by possession with Ouija Board. Razor Blade is pretty nasty too, considering the implication is that you flay off all your skin when you use it. Lead Pencil isn't quite as bad (but its still pretty bad), but the fact that Isaac keeps spurting blood from it has always kind of made me wince.

stapler is an unpleasant one for sure
apple is a reference to the mid-stage scene when isaac swallows a candy only to puke it out and realise it was a wrapped up razor
basically all items that make isaac shove something right into his skull are definitely on my list
leprosy, brittle bones, compound fracture and other disease-based items also make up the list

also I've always thought it was a bit creepy to make a 5 year old BOY become pregnant (cambion/immaculate conception)

Now that I think of it, the Kidney Stone tears aren't actually tears. They are pee explosively coming out of Isaac.

While it is worse, when you think about it, once Isaac picks up the intruder he is already dead and we are simply controlling the spider (he can walk headless after all). That means it gives Isaac a quick death, rather than give him multiple stone-sized kidney stones that blow out of him in spurts of blood.

i think the most disturbing items are the ones who try to tell you "don't worry it's still not a won run yet" and then you murder the lamb in 3 seconds without any consequences
