The clicker is a bad item dude. Not only can it rip all your hp by a lost/keeper/blue baby roll. It can also sometimes steal a passive item from you when you use it.

Aza/Judas are about all that's worth turning into over isaac in a challenge. Unless its a blindfold challenge and then maybe lilith.

Never use Clicker if you're attempting ANY achievement, including challenges.

Well, unless you are in a real pinch and most certainly dead/woulkd not make it otherwise. There is a 1 in like 15* chance to turn into Dark Judas and another 1 in 15 to turn into Azazel, even Lazarus II/Judas may be quite noticably stronger, that's like a 4 in 15 chance to get better even if an item is taken away (and sometimes, one might like to lose certain items)

*Chance based on having 14 Base characters plus Dark Judas and Lazarus II available, with an equal chance to get any, never experienced clicking into the same character and dunno in the rare case of not having a character unlocked.
Well I guess if you really want to be picky, you could use the clicker to reroll into someone else right before the applicable boss is dead, to get the achievement, but its pretty risky if you only need one character

FTFY, seriously don't know why they bothered adding this item if they were going to make it this ♥♥♥♥♥♥♥♥ later, it's explicit purpose is cheesing lost/keeper unlocks BY DESIGN and then it gets nerfed to oblivion.

i'd say at current the best use is in case you pick up a bad item and decide you don't want it later, pretty sure it removes the last item picked up.

Seriously, just because the first click turned out to be bad, it is not like it has finite uses and albeit the chance to become The Lost or The Keeper, Blue Baby may also not be to one's liking as well. Upon reading the wiki which contains some conflciting information

("Transforms Isaac into a different character, at the cost of one passive item each time the active is used." & "After transforming into all available Characters Isaac will lose one Item every time he uses Clicker.", is it an additional item? Or will items just be removed after 15+ clicks?)

It also seems like my 1 in 15 is actually 1 in 16 since one can click into the character one already.

Now simply put, this item merely may be used when a win on a streak seems unlikely, especially due to low health. Even if one becomes the Lost, one can fly and has spectral tears and as Keeper one may get back too two health rather easily while both Judas and Azazel may be what saves the run, Lazarus II as well.
