"Bag lunch/walnut/wish bone/tonsil: The requirement for taking damage to get the benefit of these keep them on the list, but all of them are interesting in their own way..."

Had a run earlier this week where I smelted/gulped the walnut. Almost every hit I dropped a new trinket and a few items. Sadly it never would drop pills, even when I held a safety cap.

Eventually it gave me the missing poster but I never got a chance to smelt/gulp it.

1: The locusts - these are pretty amazing, especially with high damage builds - conquest>death/pestilence>famine>wrath imo, though my opinion may change as I get to use them more over time. The wrath one is the only one with any downsides - namely blowing up in your face at times, which is rather painful if you don't have explosion immunity. They build up if you wipe out rooms fast enough for them not to be get used up as well, which can make the conquest one almost like a built in guppy's head that's auto used every room with enemies in it.

2: Nose goblin - amazing with a high damage build (even with low tears), even more so certain tear effects.

3: Cancer - amazing with high tears, soy milk is pretty much the only thing that can compete with the potential tear rate of this, and soy milk cripples your damage so is often less preferable (libra combo aside)

4: NO! - if I have a spacebar item I plan on keeping to the end other than the void this makes the items that spawn so much more preferably

5: Bloody crown/silver dollar - depends on the run which I prefer, but making the womb floors actually worth looking around in properly is quite neat..if only they showed up more often in depths/necro 2 rather than at inconvenient (or outright useless) times.

A few notable mentions -

fish tail (amazing with guppy's head especially - does anyone know if this stacks with the locusts or not? - guppy's head with fishtail&conquest would be insane if they do - 8-18 flies per room!)

cracked crown (great stat buff if you've been getting a lot of passive items, combos well with NO!)

vibrant bulb (great stat buff when you rarely use your active or have one that recharges quickly)

dim bulb (great stat buff when you're using a 1 room charge active - or 2 room charge ones, if combined with AAA battery)

crow heart - great for making use of ♥♥♥♥♥ of babylon with eve or other characters when you're finding too many +heart container items in a run, but also useful for actually making use of red heart drops in later floors when you have a few heart containers and a bunch of soul/black hearts. Unfortunately it also massively increases the chance of you missing out on a devil/angel room by taking red heart damage because it bypasses soul/black hearts, though if you find goat head that part can be negated and it's the only real downside to this trinket.

Swallowed penny - Keeper specific, this one is rather self-explanatory if you've ever played keeper tbh

Old School (Rebirth):
5) Petrified Poop. Not the best, but always a solid stand-by if you got nothing else.
4) Store Credit. Single use Trinket is kind of a rip, but still... Free item!
3) Cancer is great! (I can't believe the things this game makes me say. "Sure, I'll take that Placenta!")
2) Counterfiet Penny - Especially in Greed(ier) mode!
1) Curved Horn, obviously. Who doesn't want extra damage?

Middle School (Afterbirth):
Um... Fade Polaroid is cool, I guess? Maybe some others for specific characters?... I'm just not feeling the Afterbirth Trinkets, really.

New School (AB+; Spoilers):
5) Duct Tape is actually really handy. Go figure.
4) Bright Bulb / Dim Bulb (depends on whether I'm saving my active-item charge with something like D6 or Void [Bright], or if I'm using it on a boss as a weapon like Cramp-A$$'s Head or Shoop Da Whoop, or maybe also Void [Dim])
3) Bloody Crown. Item Rooms in the Womb?! Yes please!
2) Pay to Win. Reroll Machine in all Item Rooms? D6es for everybody (with $ or bombs)!

Really though. I'd drop my Curved Horn for a Nose Goblin in a heartbeat. Constant damage with all the effects for the win!

Honourable Mention:
Swallowed Penny. Every Keeper wants it, and it goes great with Greed(ier) Mode.

Sounds like you should be asking for it to get nerfed for balance reasons. It sounds much too powerful for a trinket.

Nerfing that is the last thing I want. The game needs a few OP items otherwise it would be actually literally unplayable. Nobody's petitioning to get rid of Brimstone, are they?

But its inconsistant for a trinket to outshine many of the passives in the game. You can nose goblin a boss, it sticks to them forever and just chill far away as it kills them more effectively than dead bird.

Really, Sashie, I'm done with my difficulty rants now. The community has spoken, the game has been balanced, and I'm actually having fun again. I'm still not beating every run because I shouldn't win every run. It hasn't been nerfed below Afterbirth, but it's reached the point that I think I can beat it all now and - most importantly - I acutally enjoy playing again.
My difficulty rants are over now.

It has been nerfed below afterbirth. krampus, the fallen, Angels. Now have a beam delay. Shops can now have reroll machines. Portals can be the only enemies in rooms and die after spawning a single enemy. Hush can be replaced by blue baby. Charged keys give you a key and a full charge. Half soul hearts can relace red hearts. Maggy gets a speed upgrade. I'm drowsey rips a lot of boss fights. Gulp/smelter lets you have all the trinkets. Tarrot cloth is quite OP since it works with runes like resistence.

If you only do say runs to the chest is it really harder than AB? After all, once all the achivements are done thats what most everyone still playing will mostly do.

Some things make it easier, some things make it harder. Overall, I feel it's harder than Afterbirth, but not extremely so. But again, I'm done with this subject.

Test it yourself, if you want. Uninstall AB+, play a few games, then go back to AB+ and play a few more. See if you see a difference. If you do... Tell someone else. I'm done with this.

When I was new to isaac, Fade Polaroid was one of my favorite trinkets. The chance to walk into a womb room full of enemies and them all to randomly forget about me for a while. Not so much anymore.

Pay to play/win has risen in my opinion due to reroll in every item room. Making it basically a better D6 in reguards to item rooms. I had thought it was reroll in every shop for some reason.

Bloody crown is very nice too. Its an extra two items rooms a run if gotten before womb 1. It being one of the last trinkets i unlocked I didn't get to experiense it but once or twice.
