The Cathedral and its evil counterpart, Sheol, are the next step on the progress list. The Heart isn't the end of the game (neither are those two levels by the way).
To get to the Cathedral/Sheol, you normally need to beat Mom's Heart a certain number of times, at which point beating The Womb no longer ends the game but instead allows you to progress to Cathedral/Sheol.
However, you don't NEED to do that. If you get a Devil Deal from beating Mom's Heart, you can go to Sheol- And if you get an Angel Deal, same thing, except you get to go to the Cathedral. This'll bypass the game's normal progression, meaning you can beat those floors earlier than usual before unlocking the guarenteed means of going there.
Also, I'll explain what Angel Deals are since you also seem to be new to those: Angel Deals are much like Devil Deals in that they have a chance to appear after beating a boss. However, to even get a chance at an Angel Deal, you have to refuse Devil Deals. There are of course exceptions that's a bit too complicated to go into, but generally speaking, you need to say no to every Devil Deal you see, and then you've got a pretty good chance of seeing an Angel Deal from that point on.
Angel Deals give you different items from Devil Deals, and will only ever give you one item- But, their items are free, unlike Devil Deals where you have to pay heart containers. The main advantage of Angel Deals is that while Devil Deals are a bit more reliablly good, Angel Deals have potential to give you some of the best items in the entire game, and they won't put a price on those items.
plus you can get key pieces if you bomb angel statues, after getting the polaroid and the negative
