Fruit Cake or 3 Dollar Bill?

These items seem quite similar, which one is better? Fruit cake i think is different status effects, and 3 Dollar Bill is things like 20/20 and Death's touch right? Which one is better?

The bill changes every 3 seconds if you have AB (same for the rainbow worm trinket), and can only get effects from Rebirth.
The cake changes each tear individually and has a longer list of possible effects, including some trinkets and AB+ effects (if you have this DLC).

I prefer the cake. But if you can, take both. :D

I honestly hate both of them pretty passionately. Won't pick up either one most of the time.

both are dangerous and probably should be avoided unless youre pretty good. the 3 dollar bill has great synergy with any laser type tear whereas the cake has better synergy with high tear rate.

the thing you need to avoid is the random firemind shot with 3 dollar bill and the random ipecac shot on the fruit cake. all that being said I finally beat the ultra hard challenge only because I got fruit cake and pyromaniac on the first level so I could heal by shooting walls with the random ipecac shot so I wouldnt say they are terrible

After a Fire Mind shot from Fruit Cake ended a Keeper run for me I hardly ever take it. 3 Dollar Bill is more predictable and you can prepare for a Fire Mind shot. I don't like any of those 2 items though.
