During the Suicide Mission, the team's overall "strength rating" is taken into account, as well as each of the squad's abilities and upgrades. If you've misjudged or taken the wrong person, this can result in deaths, despite their loyalty and upgrades.
Mordin isn't ideal for frontlines combat, especially if you sent one of your heavy hitters to escort the crew back to the Normandy. Considering that you've also specifically tried to get two highly skilled operatives killed, this has affected the rest of the team and their chances of surviving.

The bad thing about Suicide Mission is that you pretty much cant take Garrus with you to fight the human-reaper if you want the entire crew to survive.
As a soilder I did bring Garrus and he was useful...and everyone still lived :)
