Depending on your choices up to that point, you may not have left yourself with enough ways to accumulate enough paragon/renegade points. There may not be a way for you to reconcile Miranda and Jack on this playthrough.
(Or you could say f**k it and memory-edit more paragon/renegade points in, but where's the point of that? The whole reason why choice driven games carry emotional weight is because of the difficulties and sacrifices associated with each choice. I think being forced to choose between them actually constitues a more interesting choice for the player.
If you really do want to memory edit it, there's a program for Mass Effect 2 called the Save Editor. Google for it.)
I want both of them to live purely for the ME3 carry over, I just want as many people alive as possible and I want to romance Miranda in ME3 too.

Not related to loyalty but important
- Upgrade the Normandy's ship armor (whether loyal or not, Jack will die if this isn't done).
Important
- Don't chose Jack as the biotic specialist in the final mission (if non-loyal, someone will die. Jack is second on the list of who will die).
- Don't send Jack to escort the crew back to the Normandy (the crew will survive, but the escort will die if not loyal).
- Don't take Jack to the final boss (non-loyal party members will die).
It'll all depend on who you have holding the line in the final mission.
The party average needs to be greater than or equal to 2 for everyone for survive.
Grunt, Zaeed, and Garrus - If loyal, 4 points each.
If not loyal, 3 points each.
Thane, Legion, Samara, Jacob, Miranda - If loyal, 2 points each.
If not loyal, 1 point each.
Jack, Kasumi, Tali, Mordin - If loyal, 1 point each.
If not loyal, 0 points.
If the average is greater than or equal to 2, everyone holding the line will survive.
- If the average is between 1 and less than 2, one non-party member will die.
- If the average is between 0.5 and 1, two non-party members will die.
- If the average is between 0.0 and 0.5, three non-party members will die.
- If the average is 0.0, all non-party members will die.
The order of death is Mordin, Tali, Kasumi, Jack, Miranda, Jacob, Garrus, Samara, Legion, Thane, Zaeed, Grunt. Non-loyal ones will fall first, followed by loyal ones, in this order.
So if you do the steps I outlined, and keep your "hold the line" party average at 2 or higher, leave Jack to help hold the line and she should survive.
If you're successfully romancing Miranda, she'll survive if you take her with you to fight the final boss (she has additional dialogue with the Illusive Man if you do).
