same problem. It must be bug.
The game doesn't give you much hint of this, but wall thickness and durability make a huge impact. My underground dwarf-hold hovered around 50 - 60%, despite placing all sorts of luxury items. Then I unlocked stone walls, and placed those all around the outside. The comfort level shot right up. Then I added a second layer of stone walls up top of the fortress, for double thickness. 87%.
I then tried placing double stone walls around the lower sides and bottom of my fortress... pretty much no effect. One theory is maybe cares how much thick walls there are between you and the 'outside', and it's less concerned with walls between you and the underworld.
Another theory is that maybe fortification level is a 'global multiplier' or limiter for your max comfort level, and I've fortified enough that I've now reached my maximum comfort level based on comfort-granting items inside the fort.
Although, I do agree with a comfort level of 0% means there is a hole somewhere.. 0% = Not a completed house yet..
Also, don't waste your time with comfort items. As of right now, all those luxury items do absolutely nothing for your comfort level.
For the best percentage:
-have the minimum number of beds
-make sure all of your outside walls (floor & roof included) are brick or stone in both foreground & background
- only have as many doors as absolutely necessary.
- check corners; breaks can be in diagonal squares
- watch the totem's little dot trail trace around your house. Make sure every square it moves through is a door or on outside wall. If it passes through any other square, that square should be turned into a door or wall
- walls contained within the house have no impact on comfort level as long as every background square inside the house has a wall or window
