Armageddon achievement?

Currently in the 3rd world, and it seems to be stuck at only giving 3 portals at a time. Just curious if some maps have limitations on the portals or not (and if not, how long should I expect to run the map till the 4th-5th portals appear during the survival nights).

I made a FAQ guide, but the short version is Nightmare mode is the only way to get a fifth portal.

Actually i finally got that achievements in beta of world 4 on Hard.
