Distraint too, the subject matter of a very guilty ridden protagonist might be what you're looking for. Less possibly implied rape though.
The Hole, very, very heavy stuff, much more on the nose with its themes though.

There's a "safe" difficulty which makes it even more narrative focused, and I recommend that setting because the game's horror transcends survival elements.

A slow burner for a character-based narrative with solid writing is OMORI. It has some kiddy-ish combat elements but aren't the main appeal of the game.
