Requirements for Mode Master?

Regarding the requirements for Mode Master, I've beaten 5/6 required modes (I'm yet to do a full run of Story Mode, I'll get to that in a moment). I beat Hard and Phasing Mode with Melody, and No Return, Randomizer and Mystery with Bard, and have obtained the relevant achievements (not for No Return or Hard though, since the achievements for those are character-specific and I haven't done those with the relevant characters)
Afterwards I started Story Mode and complete the Nocturna run, but I didn't get Mode Master, so I was wondering if the achievement requires beating all of Story Mode.
I checked global achievement stats and it says 0% of players have achieved Mode Master, and the same for Very Polyamorous (all characters including DLC), I was wondering if the achievement is either bugged or it's that less than 0.1% of players have achieved it.
TL;DR Do I have to beat all of story mode or just one character's story mode for Mode Master?
EDIT: I forgot to mention my internet connection is temperemental and randomly disconnects without warning whilst I'm playing the game, it's extremely annoying, and I thought it would be relevant. I still got the achievements for Mystery when this happened though, though was unable to submit my score.

You have to beat the entire story mode. achievements show 0% due to rounding. Very Polyamorous shows as .04%(and it should be even rarer than that since there's only 30 clears for it on the leaderboard at the moment, but of course there's people that just unlock them with programs without actually doing it, so the percentages are screwed) and Mode Master at .05% on rarest achievement showcase. Mode master is probably more a .04x % or something causing it to round to .05 in the achievement showcase but to 0% in the global while some of the other .05% ones show as .1% in global.

They do both work though as I've managed both Mode Master and Very Polyamorous. Lowest of the Low is likely the actual rarest achievement for this game, but the percentage is messed up from all the people that have cheated the achievements.
