Never seen a quest marked "failed" in the whole game, weird.
Sometimes if a game does something cool for the sake of it's plot it's best to break out of a completionist mentality and ride it out.
It's tricky to do, but I feel like 100%ing games becoming the norm really has disincentivized twists, turns and risky storytelling. It's kind of cool that a whole city you can quest in can be just wiped off the map permanently if you're not paying attention to the growing threat as you journey around the Emerald Stair.
If you reload to before you complete the ruins you should be able to prevent it happening by doing some side quests relating to the Steel Garrote. Up to you!
You explained it best. My issue isn't that I made a choice that resulted in a quest failing. It's that the game makes it seem like that bounties are achievable regardless of decision making by having every map have a completion list. Seems odd design to force a 4/5 for Bounties. It's no different than making a fast travel point inaccessible permanently after a choice you made and forever displaying 13/14 fast travel locations on the to-do list.
My post is specific to a particular bounty that is not achievable even after the bounty hunter relocates.
