But there are Veteran Server admins community helper on Keen Discord in the "Help-for-community-server" channel, they are helping every days and are very kind so that is good place to ask for help.
One of the Veteran helper shared this important information that we added in the pinned important of the help channel that I will paste here cause it may be the answer you are looking for :

Settings for servers need to be identical for the main spaceengineers-dedicated.cfg and sandbox_config.sbc in the save folder
I never play with auto respawn, I prefer to use Medic rooms shared with all so all new players can join there.
I remember having this option in solo and when I did, I could choose which place I respawned to. I'm all for sharing medic rooms but if someones survival kit makes me respawn there, it's difficult.
Last time I asked the guy to turn off his survival kit. And sure enough I spawned back at the base. lol. But disabling auto respawn gave us a choice when we played multiplayer in a solo game (if that makes sense) so not on a server
But I never go MP just to build cause that I can do Solo, I go MP in Premade PVP worlds only,
like : Race Track scenario, Battle for the ores, Dogfight arena, etch ...
= This way I can add only one Medic room shared with all so all will join where the PVP play is.
<EnableAutorespawn>true</EnableAutorespawn>
and set it to
<EnableAutorespawn>false</EnableAutorespawn>
in BOTH the sandbox.sbc in your savegame AND the SpaceEngineers-Dedicated.cfg of your server.
Be sure you shut down the server before you change those files and restart it afterwards.
That should do it.
So you may do the same and copy/paste the files content of that Local tested save to your server save, so replace the file content for the local files if you use rented server.
What I mean is create your world Local, test it and use it.
Yes if we enable auto respawn with respawn distance option but not working if disabled.
While on the Mars planet, we have a base with 2 med rooms. I was in space, died, and respawned next to his ship side by side with his survival kit. I had no option, just spawned there.
His ship was closer to me than the med room on the planet. So yes, I guess the game chooses the thing that is near you.
If Enable Auto Respawn is on (true) that would be the default thing. If auto respawn is off, you get to choose. @Jel is right, I need to change 2 things. I will try later and report back.

Both have this option:
<EnableAutorespawn>true</EnableAutorespawn>
I've changed it to: false
And there's this file:
SpaceEngineers-Dedicated.cfg
It's located in:
C:\Users\yourname\AppData\Roaming\SpaceEngineersDedicated
It also had this option and showed 'true'. Changed it to 'false' as well.
Now all I have to do is respawn. But I'm in space at the moment and if this fails well.... I don't want it to fail. So I land first.
EDIT:
Face ... desk ... On the dedicated server config panel, there's the tab 'Players'. This tab has the option "Enable Auto Respawn". It's actually pretty easy...
So yeah, there's that.
