How do you revive a dead party member? Can't seem to figure it out.

Hi, when a teammate gets knocked out, a quest will be added and his soul is supposed to appear on the map.
But we've just found out we have a critical bug that the soul is not spawned in certain circumstances, which can quite be common also. We'll make sure we tackle this one ASAP.
