Id suggest you get the content manager for easier setup and better access to all features the game has.
I found that this issue was due to the shifter not being configured within the Content Manager. That is, the H-shifter settings were blank. The solution was to configure the
H pattern shifter in Content Manager in Assetto Corsa|Controls|Buttons section. Tick the "Use H-Shifter" option and configure the buttons for the device you have.
Once this was done the shifter worked fine when racing with a car using a H
pattern shifter within the game.
The shift paddles were already set up in Content Manager and never had an issue and still worked fine.

1. The most glaring of all, are you actually driving a car with a H shifter or did you just jump in to a modern day Lambo trying to use your manual shifter?
2. In control manager > controls, make sure you have ticked the box "Use H-shifter". It's on the same tab where you assign your gears.
3. on the Drive tab in CM, click the 3 dots on the left next to "tyre blankets" and UNTICK the following:
auto shifting
auto clutch
autoblip
