Town health dropping
I have two herbalists, a hospital just over 4k food, more than enough firewood and coats, and my town (with just over 50 people) is down to two hearts and still dropping. What am I missing?

How many herbs do you have stored? Do you have an herbalist near most of your houses? Do you have variety in your citizens' diet?

Well I can't find herbs stored anywhere so I would assume they're used as soon as they're brought in, so I should make more herbalist. And their diet is probably 85% beans and 15% fish. I didn't know variety mattered, so I should make a few of my farms grow something else?

Your citizens need a variety of food to stay healthy. You need protein, grains, fruits, vegetables, carbohydrates... plus herbs to make up for anything they lack in diet.

Yes, food variety actually does matter in this. Three or four types is sufficient for good health, in my experience. It's pretty easy to satisfy if you also add in a gatherer's hut in a prime area.
