Why can't i hit full health?
I have two herbalist huts with 2 workers each and hundreds of medicine stored, a hospital with a physician, around 6,000 food all year - (fish, chicken, eggs, all gatherables, venison, chestnuts, apples, pears, peppers, squash, potato, cabbage, walnuts, pecans), plenty of firewood, wool and leather coats, 80% education rating, every house is a stone house. I have no idea why the hearts arent at a full 5. And when clicking on individual citizens they all have 4 hearts regardless of age. Whats up?

Have a chapel? Graveyard?
Try watching a houses inventory and seeing what food a family eats, lack of food variety seems to be the biggest health issue. Even though you have all these food types in stock, they may not be being evenly distributed to your population
I have two markets that distribute the food quite evenly. Also have 3 graveyards that just now filled up from old people.

I think the issue is markets, setup some markets and get some variety in their diets.
Is your average at 4 1/2? i occasionally find some of my people below 4 1/2 but my average USUALLY stays at 4 1/2.

Sometimes i have noticed when this drops disease my follow. Typicially this occurs when accepting new nomads or a merchant stops by and i trade with him rather than dimissing him. Because sometimes these people seem to carry the smallpox.

Also other things affect their health too like how long they are staying outdoors before going to heat up at a house in winter. Even if not labeled *freezing* they can lose health if staying out too long, It only informs you they are freezing when it gets extreme.

Same with eating, they can Lose health when they take too long at a job site and have to run back to eat. The game will only tell you when they are Starving and in danger of dying.

So the proximity of their House, to their current job, affects their travel time etc and time away from things they need. When they are "going to get something to eat" and are leaving their place of work they can lose health on the way to do this. Things like this also add up.

There are ways to HELP this like keeping houses closeby BUT unfortunatelly since we cant actually assign certain citizens to specific houses and the game does this auto, they can be forced into a house far away from theuir place of work. Thus making this out of your control you can only Mitigate this, but not fully stop it.
your people hate to have you in their face all the time, and only wish you to shut up for a while and provide them with some free grog.. they also hate you for forcing them to read that whole tirade that rough_divide wrote while they could have collected some mushrooms instead.

for full health you need every posible food in production and medical herbs with hosbital and they need to be avaible for predy much all ppl in village market also helps towards this in middle of village centers

As others said, increase your food variety supply (maybe some of your types are in small quantities), and make sure your citizens can get to the herbalist when they need to. That, and just wait. It can take time for their health to go up.
I have all hearts but my last is at half. Reading this post makes me wonder if I need to give them "warm coats" to get the last heart full.
