I think that's what the plot is.
About the cars, I imagine that they represent negative memories manifesting the guilt and traumatic experiences as vehicles as a way to torment our character since that was the last thing they saw before getting locked in the trunk.
I'm pretty sure Jose is the protagonist's son, and Frank is the father of Andrea. Frank was the one quoted as saying "it's never been the same since my wife died." There's also a note near the end of the game that implies she's his daughter. I guess we'll see what the new update brings though!
Mainly because I wasn't sure why she killed her patient. Did the father trap her in the basement with the daughter until she was better?
How did she get out of that basement after killing her? What's the deal with the car hunting down both her and frank? Are they both just being punished for their guilt like Silent Hill?
