any configuration of above two categories will be your game
Is it possible to copy (duplicate) your save game, so you leave your (finished) normal game in one slot and you start the new game plus in the other slot? Has anyone tried something like this?

Ok, I have figured it out. Here is what you need to do in order to duplicate your save:
1) Start a new game (Play campaign > New Game)
2) After first checkpoint - close the game
3) Go to the save file location ({...}\Steam\userdata\{...}\239140\remote\out\save)
4) Copy your 100% save file (in my case: save_coop_0.sav)
5) Overwrite the save file created in step 2 (in my case: save_coop_1.sav)
6) Launch the game - now you can start the NG+ from the other save slot
