whats the difference
between campaign and the following, and which should i start with.if it matters storywise

Campaing is the main game story. And the Following is a DLC.
You should start with the Campaign first because the following kinda continues the story.

Start with the main campaign. It's not recommended that you go into The Following unless you're level 12 or higher. The Following is "harder". If you start The Following now, you'll spoil parts of the story for yourself. My advice to you is that you get 100% story on the main game first before you enter The Following.

The difference between the main game and The Following is that they're in two totally different maps.

It's not even possible to play the DLC first. You have to at least finish the main game prologue, but they recommend you level up to at least Survivor 12 first. If playing on Hard or Nightmare, best to beat the main game first, and in the case of Nightmare, have all your skills maxed out.

Also, besides being more difficult in gameplay, the Countryside and cult of The Following, is a story told to you by someone in the main game, so it's not only about a cult "following", it pretty much follows the main story.
