There's 3 mechanics that affect the overall difficulty.

2. Difficulty.
3. Story %.
If you combine all of this, the outcome will be hell. The normal weapons you find without upgrades or crafting will take around 10+ basic hits to kill a normal biter, assuming you have 25 Survivor skill-tree and full support from Legend skill-tree on the current type of melee weapon you're using. Also, the unlimited stamina from Power and Agility skill-trees no longer work on Nightmare mode, forcing you to micromanage it by drinking boosters for effective combat.
For best combat efficiency on Nightmare Game + at 100% story, you need best possible weapons, ones that are obtained at Survivor skill-tree rank 25 and with support from Legend skill-tree, also they'll need upgrades and blueprints, now keep in mind that weapons break down quickly, draining your cash, upgrades and materials if you're not looting.
Also, you can already disable HUD in game so you won't need to use mods for that. You can disable crosshair as well.
Don't worry, the difficulty remains way ahead even for maxed out character at Nightmare Game + (100% story), you'll have real hell winning against evolved volatiles.
Quite a massive overhaul ~ weapons, TOD cycles, monetary system zombie AI, etc., with two modes ~ Casual and Survivor. Survivor looks to be right along the lines your looking for. I'll post the link a bit later if you haven't the time to look it up.
