Yes, starting NG+[dyinglight.wikia.com] on Hard mode, is sure way to get it. Just don't downgrade the difficulty to Normal until you reach 100% story completion and never join others, these are rules for 100% chance of getting the Reward outfits.
I'm not sure, I don't recommend this. I personally never tested it either.
There wouldn't be any problems if they join you, regardless of which difficulty they come from, you will get the Rewards as long as you follow this rules: 1. Never join others, until you get 100% story. 2. Never downgrade the difficulty until you get 100% story.

Yes, most likely. Game will warn you about joining others(I think), but be careful about it. I personally have never tested it, therefore unfortunately I cannot give precise information regarding this.

For him, I don't think so I'm not sure, but it doesn't matter affect you. Also I think he has to start his game on NG+ Hard to have the ability to unlock the outfit, but I'm not entirely sure.
