It's related to the red and blue influences you pick on the Map Device.

You get the Eldritch Embers and Eldritch Ichors, which allow you to add 2 implicit modifiers on armor. The armor cannot be Influenced or Unique.

"If The Searing Exarch is dominant, remove a prefix modifier" means: if the Exarch modifier has a higher tier, one of the prefix mods will be removed randomly. A regular Orb of Annulment will just pick any random modifier.

You can use this currency and others like it to increase your chances of getting certain outcomes. So basically better & less risky crafting, but at the cost of needing to get the special currency.

To see a breakdown of what mods on your item are prefix and which are suffix, hold the default hotkey alt while viewing their tooltip.

If this for some reason doesn't show, look for the advanced tooltips checkbox in the options, it should be enabled.

Rare items have in general 3 slots for suffixes and 3 slots for prefixes. If one of those slots has an undesired mod in it, you might want to remove it and replace it with another. Like a low % inc phys damage roll on a weapon or so, which otherwise bricks the item. Often, you'll have a desired mod for all of those slots, and with such crafting techniques, you get another attempt at fulfilling those desires.

Eldritch Orb of Annulment will delete that bad mod hopefully and put in anotehr mod hopefully?

The orb of annulment will not add modifiers, that's for other currency or the workbench or other crafting techniques to do.

And whether it's chance based depends on how many choices it has to remove a mod. If only 1 mod fits the criteria, it can only remove that mod, hence it would be deterministic in that case.
