Why i failure the ultimatum?
sorry about my poor English skill.

i failure the ultimatum 3 time that I didn't die. and all 3 round is just survive no protect altar no killing monster but I failure. I never go out from area and always choose phantom and trap challenge. there are no any sign about failure. Does anyone have any clue why this happens?

If you get 7 ruin, you'll fail. So don't get hit by the monsters from that mod.

You said Phantom Challenge? He stacks Ruin on you.

Restless ground 3 also has ruin effects.

That? Any of them? Ruin has multiple mods it gets added to the fight with.
"Ruin" and "Stalking Ruin" have it at all levels, "Restless Ground" brings it to the fight with Rank 3 of the mod.

Many patches of Unhallowed Ground will appear that Hinder players and cause slain Monsters to resurrect as Ruin-inflicting Phantasms. Fail on reaching 7 Ruin

There are three difficulty modifiers that apply a stacking debuff called ruin:

One is a slow phantom that follows you and applies ruin on hit.
A second one applies ruin to a chosen white enemy that spawns, also on hit.
Third is the revive ground effect - on the third tier, the phantoms that spawn when you kill enemies in the ground effect also apply ruin.

Ruin persists between rounds and 7 stacks mean you fail the ultimatum.

If you want to be very sure never to lose without actually dying, you choose none of those three.
Phantom is only an issue for builds that don't or can't move while clearing (mostly channeling or "stand your ground" kinda builds).
The ruin on white mobs one typically only wrecks people that can't consistently apply damage across all or most of the arena (some melee builds can get touched by those if they spawn in behind them, same for some summoner builds without ways to apply damage until a minion decides to smack em).
Revive ground effect is, at least to me, the most dangerous one because it can spawn a lot of minions that have ruin on tier 3 and they're ranged. On maps where attacks chain or get multiple projectiles, those can wing you even if you're usually fine.

Ruin is one of the easiest to deal with imo except like others said the phantasm ones at tier 3 on the restless ground mod as it spawns a lot of them. Just don't stand still for too long + watchout out for blue effects and its easy to avoid failing to ruin. The only easier ones (except the freeze orbs with freeze + chill immunity flask which is standard to have) are conditionally easier. Razor dance if you have a corrupted blood immunity from a corrupted implicit on a jewel or miasma if you have decentish chaos resistance (would say around positive 35% or greater) ones become easier than any others with those. Fire one is also not to tough if you move alot but can do a sizeable amount of burst if caught unaware and take multiple hits at once.

The truly hard overpowered mods are ones like limited active flask to 1, escalating damage or escalating cast speed + attack speed which you should avoid at all costs in probably every build if possible because they are very nasty.

True. If you're almost constantly moving, the other two ruin modifiers are pretty easy, free mods.
