Game runs smooth as butter, no fps drops at all.
So i suggest to disable AA and shadows (or lower them) also post processing, and see if it helps
Given the information it sounds like you have lock step on and if you are dropping packets or have latency issues it will stutter your game.

Still game like to freeze then speedup and fps drops. Poe has really bad optimalization
freeze and speed ups are the main indicators for network issues, either yours or GGGs, while playing on lockstep network mode.
And yes, the game optimization on poe is notoriously terrible. Almost every new league mechanic creates freezes and crashes once it goes core. The game also creates extremely laggy instances every now and again, just yesterday i got a pretty much unplayable sirus instance and the lag vanished as soon as i opened a map...
GGG staff have some seriously talented people when it comes to imagining the game, but the programing section really sucks at their job
