How to Get Portal Gem?

I just got a Portal Gem, I think I dropped from a monster, or reward for a quest? I'm not sure, but surely is great to be able to teleport town without worrying about the scroll. I search for how to get this gem on Google and it told me is by trading, but I'm 100% sure I didn't get it by trade, this means the wikia isn't up to dated:

So... How can I get this Gem besides trading?

It's a super rare drop by itself (as in I found 1 on my way to 40 challenges this league), the best way to target it IMHO is to farm breach content for The Realm divination card, which your item filter might hide, especially if you're using a stricter variant. That should yield you a portal gem pretty quickly, if you're in maps already.
