Even though you somehow come to the conclusion you aren't stunned, I still suspect that might be the cause, as the triggered curses from your Vixens still apply. Try speccing into Unwavering Evil which your Cluster Jewel enables you to. (Just having your Cluster Jewel Socketed will not grant you the bonuses listed.)
Had the same "sometimes off" experience when switching between having and not having that notable...
Stuns don't even have to be that long, so you might overlook them.
The other potentially possible reason for bane to not trigger but the casts from the Vixens to go through that comes to mind would be animation cancelling for spells at a very precise moment? Dunno if that is even a thing.
And its sad but the truth, "Animation cancelling"!!
Yes, if you cast Bane and immediately click "walk" afterwards, it looks like you've just casted Bane but you didn't, or at least you didn't finish. So holing the left mouse button apparently is essential!

Are you on predictive or lock stop because i never run into that issue with my bane despair.
Bane has a cast time of: 0.60 sec
So if you are clicking as opposed to holding down your movement button you will interrupt your casting.
Ah the effect of trying to go too fast XD
